Assuming everything is true as reported....these days with the level of production automation (so you spoil like 100 or so by a lathe misalignment before you realise), it was probably a bad production run batch that for whatever reason (vodka + "good enough" + russian standards of quartermastering and certification etc.) made it to frontline.
